Presenting Concerns & Context

School counselors: use presenting concern language rather than diagnostic codes unless a formal evaluation has been completed.
Strengths-based documentation is required in many settings and improves outcomes. Lead with assets.

Treatment Goals

Write goals in SMART format (Specific, Measurable, Achievable, Relevant, Time-bound). Up to three goals. Leave unused goal cards blank — they’ll be omitted from the printed output.

How to use this template

Fill in the browser, print to PDF. Everything stays on your device. Use a client identifier rather than a full name. The resulting PDF becomes the plan of record in your chart system — Sandstories doesn’t store anything.

You don’t need all three goals. One strong, well-written SMART goal is better than three vague ones. Leave Goal 2 and Goal 3 blank and they’ll be omitted from the printed output.

Review dates are not optional. A treatment plan without a review date is a plan that never gets reviewed. The first review date and the mid-point date protect the client and protect your practice.

Documentation reminder

This is a scaffold, not a legally complete treatment plan. Your jurisdiction, license, employer, and insurance contracts may require additional language, signatures, or formats. The SMART goal structure and sandtray-specific intervention language are provided to save you time — but this template is a starting point, not an endpoint. When in doubt, consult your supervisor and your liability carrier.
